About Us
Earlsfort Health is a healthcare design and governance company specialising in elderly and acute hospital care. It was established over 20 years ago to delivered novel, and bespoke health system re-design, and clinical governance solutions to the public and private healthcare sector in Ireland and abroad. Over that time, we have successfully partnered with the HSE, Mowlam Healthcare, Charter Medical Group, Mater Private Network and many other nursing home providers.
Our focus on elderly and acute care differentiates us from other healthcare companies who have concentrated on elective surgical and medical care. We are uniquely placed therefore to support health systems meet the inexorable demand for elderly population medical care in the community and hospital setting.
Our Clients
Emergency Department. Mater Private Network
We supported the development and launch of Mater Private Hospital Emergency Department in Dublin. We continue to manage the consultant governance and staffing of the facility which delivers complex medical care to over 10,000 unscheduled presentations each year.
Minor Injury Units (MIU). Charter Medical
In partnership with Charter Medical and HSE we developed MIU services in North Dublin and Mullingar. We continue to provide consultant governance and day to day clinical support to these services which manage over 30,000 ambulatory care patients annually.
Level 2 Rehabilitation Care. Mowlam Healthcare
In 2024, together with Mowlam Healthcare (one of Ireland’s largest nursing care home providers with over 1800 beds currently in use across the country), we opened a 25 bed Level 2 rehabilitation facility within a 75-bed nursing home in Caharcalla, Ennis, Co Clare. This facility accepts early discharge patients from University Hospital Limerick (UHL) according to HSE protocols thereby reducing the average length of stay for UHL. The model has been sufficiently successful, that HSE commissioned a further 15 rehab beds which opened in November 2025. In addition, we now manage a 25-bed rehabilitation/step down facility in Leopardstown, Dublin to support St Vincents University Hospital.

Real-time monitoring, analytics and AI insights, provide healthcare professionals with comprehensive patient insights, enabling proactive intervention. With robust protocols and risk mitigation strategies, connects.health upholds the highest standards of clinical governance and patient safety. The platform allows for healthcare facilities to expand into step-down or rehabilitation services without additional infrastructure.
